The credit card signature (online purchases are considered by Star Tech Health as signed contracts although a formal signature may be requested of the signatory by the Star Tech Health staff) hereby commits to contracts for material/s or services according to all terms and conditions stated herein. If payment is by credit card, this is an express agreement for the charges stated above by the authorized cardholder. By signing this credit card card/charge authorization, or by forwarding this credit card/charge authorization by electronic methods, customer waives any and all rights to charge-back the balance authorized for materials and or services rendered, and further agrees to pay all fees, charges or costs incurred by Star Tech Health Services, LLC by reason of denial of customer's charge account or credit privileges resulting in non-payment of the balance charged.
